Please fill out the form below in order to schedule an intake appointment, during which we can discuss what you are seeking in therapy and see if we feel ourselves to be a good match. I am also happy to provide you with other therapist referrals upon request.
Sessions and Fees: Sessions are 50 minutes in length and scheduled weekly unless otherwise agreed upon, with a standard rate of $200 per session. Sliding scale rates are available upon request.
While I do not currently accept insurance, I will be glad to provide you with an invoice for reimbursement if your insurance plan allows you to work with an out-of-network provider (PPO) plan.
Forms: Before we meet, please print, read and fill out the Office Policies & Informed Consent and Client Intake forms below. Please bring these two completed forms with you to your first appointment, as this will save time in our initial session.
